Make a meaningful impact as a Part-Time Supported Independent Living Worker with Community Living–Central Huron.
Your role will support individuals in living independently and accessing community resources.
Community Living–Central Huron seeks flexible individuals to provide personalized support to those living independently. With a focus on relationship-building, your duties will include developing individualized support plans and coordinating with various partners. You will address emergency situations while promoting choice, advocacy, and inclusion.
Key Responsibilities:
• Provide flexible support to individuals living independently
• Develop person-directed plans and strategies
• Act as liaison, coordinating with internal and external partners
• Promote access to community resources and natural supports
• Maintain documentation and compliance with policies
Requirements:
• Ontario Secondary School Diploma or equivalent
• Minimum 2-3 years experience in Human Services
• Strong communication skills, verbal and written
• Knowledge of provincial and federal legislation
• Valid Ontario Driver's License with vehicle access
