Location: Southwestern Ontario
We are hiring a Social Worker for Discharge Planning on a casual part-time basis in Chatham-Kent. This opportunity is suited for professionals with a BSW and solid clinical experience.
As a Social Worker specializing in discharge planning, you will support clients through transitional phases of care. This casual position requires 3-5 years of clinical experience in social work. A BSW is mandatory, and an MSW is preferred for a competitive pay rate.
Key Responsibilities:
• Evaluate discharge planning requirements for clients
• Collaborate with various healthcare teams
• Facilitate access to community resources for clients
• Maintain comprehensive documentation of services
• Stay informed about local agencies and resources
Requirements:
• BSW mandatory; MSW preferable for higher compensation
• 3-5 years clinical experience required
• Current membership in the College of Social Work
• Familiarity with Chatham-Kent support systems
• Proficiency in Microsoft Office tools
