This full-time position requires collaboration within an interdisciplinary team to assist residents dealing with mental health and substance use challenges. You will guide tenants through personal service plans, focusing on recovery and building therapeutic relationships. Key Responsibilities:

• Work closely with residents to craft recovery-oriented support plans

• Facilitate connections with essential community resources

• Manage case coordination with a focus on resident health

• Handle resident complaints with care and efficiency

• Conduct group programming and orientation for new residents

Requirements:

• Bachelor’s degree or relevant experience in support work

• Case management experience with vulnerable populations

• Willingness to complete a recent Police Vulnerable Sector Check

• Valid driver's license and access to a vehicle

• Current Standard First Aid & CPR Certification or willingness to register
