The Thames Valley Family Health Team is one of the largest family health teams in Ontario. With more than 120 physicians across 16 sites within London, Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive inter-collaborative primary health care to over 162,000 patients.
Thames Valley Family Health Team is seeking Full-time Nurse Practitioners (NP), with either a Paediatric or Primary Care designation, to join our dynamic team. Dedicated to optimization of full scope of clinical practice, our organization is looking to mentor a new NP, or experienced NP into this unique clinical area. The focus of this NP role will be in providing care through the rapid assessment of emergency department diversion clinics and an NP
- Led Virtual Urgent Care (VUC) Clinic.
Requirements
- Successful completion of a master’s degree in nursing
- Current registration in the extended class and in good standing with the College of Nurses of Ontario
- At least 2 years of recent nursing experience with a clinical focus in primary or acute care: pediatric experience preferred.
- Current member of the Nurse Practitioner's Association of Ontario (NPAO) or Registered Nurses Association of Ontario (RNAO)
- Current Basic Life Support for Healthcare Providers course: BLS-HCP (C)
- Well-developed clinical reasoning skills and ability to work both autonomously and collaboratively
- Clear understanding of NP Scope of Practice
- Must have access to reliable transportation as travel between sites may be required
- Availability to work 12pm – 9pm in a hybrid work arrangement with availability Monday through Sunday and on holidays
- Improving access and flow by working to full scope with the focus on preventing unnecessary ER visits and/or decreasing the length of stay of ER visits
- Working to full scope by diagnosing, prescribing medications, ordering diagnostic tests and imaging, and making specialist referrals, and coordinating care with other subspecialists
- Ongoing education, counselling and advising patients/families and other care providers.
- Liaising closely with community partners, including 811 (previously Telehealth)
- Identifying opportunities for quality improvement including the development of policies and procedures; patient care initiatives that reflect best practices and relevant Canadian guidelines in the delivery of Emergency Room and Virtual Care
- All duties and responsibilities of the NP conform to professional practice standards of the Thames Valley Family Health Team.
- Recent experience collaborating with external agencies and community partners preferred
- Ability to establish effective interpersonal relationships with colleagues and therapeutic relationships with patients and families
- Expertise in critical thinking, clinical problem solving, negotiation and conflict resolution
- Demonstrated excellence in interpersonal relations and communication skills, both oral and written
- Demonstrated excellence in clinical practice
- Demonstrated practice and commitment to the principles of patient & family centered care
- Demonstrated skill in insight, self-awareness and self-management consistent with high emotional intelligence
- Demonstrated ability to express oneself assertively and effectively in a non-judgmental and respectful manner
- Demonstrated experience and leadership in education and research
- Ability to function as an independent practitioner and a collaborative team member
- Proven ability to act empathically, while establishing and maintaining productive relationships
- Ability to be flexible and adapt positively to constantly changing clinical situations and multiple demands
