* Qualifications / Requirements:

*** MD/DO
* Unrestricted State Medical License
* Unrestricted DEA License or ability to obtain
* BC/BE (ABP)
* Completion of ACGME Pediatrics Residency program
* Board certification required within 5 years of eligibility
* BLS
