- Provide senior clinical leadership in assessing and managing older adults with complex needs

Job description Job responsibilities- Provide senior clinical leadership in assessing and managing older adults with complex, multi‑factorial needs
- Deliver and promote timely, safe and person‑centred care across acute, ambulatory and community pathways
- Work collaboratively with multidisciplinary teams to guide clinical decisions, prioritisation and care planning
- Contribute to teaching, supervision and development of junior doctors and wider team members
- Lead or support quality improvement, audit and service development initiatives
- Model compassionate, inclusive and professional practice that upholds dignity and respect
- Apply strong communication, teamwork and clinical judgement to enhance safety and patient experience
- Support effective coordination of care and continuity across services to improve outcomes for older people
