Join Greenwich Health Ltd as a Senior Clinical Pharmacist within the Greenwich GP Federation. The role will be based in the Riverview Health PCN, a group of 5 practices, and involve working across a wider community to support pharmacy and medical teams in delivering high‑quality, patient‑centred care.

As a key member of the multidisciplinary team you will:

• Manage patients with co‑morbidities and chronic disease;

• Provide support to general practice staff on prescription and medication queries;

• Lead medicines optimisation, quality improvement and hold aspects of the Quality and Outcomes Framework;

• Deliver patient‑facing clinical medication review clinics and telephone support.

• Provide patient‑facing reviews and medicine support.
• Manage medications in hospital discharges and long‑term condition clinics.
• Offer real‑time telephone support to patients and practice staff.
• Act as a medicines information centre for the practice.
• Adhere to repeat prescribing processes and oversee medicines safety.
• Lead quality improvement projects and support the Quality and Outcomes Framework.
• Maintain professional indemnity and authorisation for prescribing changes.
• Support health and safety, equality and confidentiality obligations.
• Participate in training, performance appraisal and professional development.

• Masters degree in Pharmacy.
• Post‑graduate diploma or equivalent training/experience.
• Independent Prescriber qualification (desirable).

• Minimum 5 years post‑qualification experience, including therapeutic and clinical knowledge.
• Experience in general practice settings and understanding of GP team structures.

• Registered with the General Pharmaceutical Council (GPhC).
• Membership of the Royal Pharmaceutical Society (desirable).

Salary: £48,840 to £56,610 per year
Permanent contract – flexible working within a 40‑hour week.
