Job Description

We are looking for qualified, experienced Adults Social Workers with knowledge of adult social care legislation, principles and best practice. You will have experience of assessing the needs of adults and or their carers and the management of risks, including safeguarding enquiries.

You will be committed to working creatively within the Complex Care Team using The Strengths Based and Person-Centred Approach to enable our residents, who meet eligibility criteria, to experience personalised, integrated care. This role also requires you to support the resident in delivering a high quality, robust, efficient and effective care support that maximises individuals’ achievements and enabling independence of residents in collaboration with their family/carers and partnership organisations to support a strengths-based approach that minimises the need for long term services.

We will support those needing to complete the ASYE programme if you have not completed one previously. You’ll be responsible to the Team Manager and be able to fully contribute towards the improvement and efficiency of the Team.

Responsibilities
  • Assess the needs of adults and their carers.
  • Manage risks, including safeguarding enquiries.
  • Work creatively within the Complex Care Team using The Strengths Based and Person-Centred Approach.
  • Support residents in delivering high quality, efficient and effective care that maximises independence.
  • Collaborate with family/carers and partnership organisations to minimise the need for long-term services.
  • Report to the Team Manager and contribute to team improvement and efficiency.
Qualifications
  • Qualified and experienced Adults Social Worker.
  • Knowledge of adult social care legislation, principles and best practice.
  • Experience assessing needs and managing risks, including safeguarding enquiries.
  • Ability to work within a Strengths Based and Person-Centred approach.
  • Experience of supporting residents to maximise independence.
