Job Summary

We are seeking an enthusiastic Physiotherapist or Occupational Therapist to join the Community Independence Service (CIS) and support older adults in Hammersmith & Fulham to live independently. You will join a committed, multidisciplinary team delivering high‑quality patient care across the Home First/Discharge to Assess pathway. Working alongside therapists, healthcare assistants, coordinators, Rapid Response, Reablement and Social Care colleagues, you will support safe and timely discharges into the community.

The service is co‑located with LBHF Adult Social Care and works closely with Charing Cross Hospital and other local acute partners. You will also be connected with peers across Hammersmith & Fulham, Westminster, and Kensington & Chelsea, contributing to service development and partnership working.

This role offers the opportunity to make a meaningful difference through face‑to‑face care within a supportive multi‑agency team. We actively support mobile and flexible working.

Main Duties
  • Carry out day‑to‑day care of patients within their own home environment, including personal care, medication prompting, toileting and meal preparation.
  • Perform safe moving and handling of patients using equipment prescribed by Occupational Therapists and Physiotherapists, such as rollator frames, Sara Steady, Re‑turn and hoist.
  • Review and adjust basic equipment that has been prescribed and installed by Occupational or Physiotherapists, such as mobility aids or toilet frames.
  • Take observations (e.g., blood pressure and temperature) after completing the appropriate competency, NEWS2 scoring and early identification and escalation of deteriorating patients.
  • Participate and contribute to audits, service development and projects associated with improving the quality of service of CIS.
Key Requirements
  • Degree/Diploma in Physiotherapy or Occupational Therapy.
  • Registration with the HCPC as a Physiotherapist or Occupational Therapist.
  • Previous experience working in the UK or a similar health economy as a Physiotherapist or Occupational Therapist, including older people/frailty assessment, medicine, general surgery, orthopaedics, falls, respiratory, neurology, long‑term conditions, teaching juniors, and audit/service development.
  • Excellent IT skills (Outlook, Word, Excel, clinical records databases) and strong communication skills, fluency in written and verbal English.
  • Demonstrated flexibility, problem‑solving, initiative, and leadership skills.
  • A commitment to working with older people and understanding of disability and its effects.
  • Understanding of NHS service delivery, CPD, standards of practice and evidence‑based guidelines.
  • Ability to work under pressure, set priorities, delegate, manage own time, and work effectively within a multidisciplinary team.
Desirable
  • Student placement facilitation.
  • Goal setting and frailty/older people's rehabilitation expertise.
  • Experience working as an autonomous practitioner in a community setting.
  • Experience teaching students.
  • Insight into own strengths and development areas relative to the Trust’s values and behaviours.
Working Hours and Pattern

The service operates 7 days a week, 8 am – 8 pm. Hours for therapists vary within these times. Full‑time permanent contract.

Pay and Band

Band 6 – Salary £46,419 to £55,046 per annum pro rata, including 20% HCAS.
