About the roleThe Pharmacy team at Pulse is seeking a dedicated and experienced Band 6 or 7 Specialist Cardiovascular Pharmacist to join our team. This position provides a fantastic opportunity for a skilled professional to significantly contribute to our pharmacy services.
Duties and Responsibilities- Medicines Reconciliation :
Ensure accurate and complete medication histories for patients. - Ordering & Clinical Review of Medicines :
Manage and review medication orders to ensure optimal patient outcomes. - Facilitating Discharge :
Screen, dispense, check, and counsel patients on their medications to ensure a smooth discharge process. - Participation on Ward Rounds :
Collaborate with the healthcare team during ward rounds to provide expert pharmaceutical advice.
- Up to date GPhC registration.
- Relevant Pharmacy qualification.
- Hospital experience as a pharmacist.
- Strong communication and interpersonal skills.
