An exciting opportunity has come up for the Adult Community Nursing Service. Band 5 Community Nurses will work across the Hounslow areas, based at the Heart of Hounslow Health Centre. The service operates from 0700 until 1900 seven days a week, working alternate weekends, with three shifts: 0700‑1500 (early), 0830‑1630 (day) and 1100‑1900 (late).
Collaboratively working with our integrated rapid response teams, community matrons and social services colleagues is essential to ensure seamless care for our patient group.
Provide high‑quality, skilled nursing care and assist the Community Nursing Team Manager in the planning, implementation and evaluation of the holistic needs of patients, families and carers. Deliver high‑quality nursing care to housebound patients with complex needs to enable them to remain in their own homes, working closely with specialist nurses, allied health and social care professionals to avoid unnecessary attendance in A&E or hospital admission.
This role involves working independently, but as part of the Community Nursing Team covering a particular geographical area. It will include first assessments and reviews, planning, implementation, monitoring and evaluation of individual programmes of care. Candidates will be expected to be flexible to support the corporate approach to patient care as and when required.
