OverviewThe post holder will be hosted by the Camden School Nursing Team but will work on a day-to-day basis with the Camden Youth Justice Service, based at Crowndale, Camden. All work activity will be overseen and managed by the Service Manager for the Youth Justice Service, with additional support and supervision available from School health colleagues. The post holder will engage with children, young people, carers, and families to ensure the physical wellbeing and health needs of this client group are identified and responded to in a timely manner.
Mainduties of the job
The aim of the post is to develop a universal physical and mental health offer for children known to the Youth Justice Service (YJS) and children and young people (CYP), aged 10-25, at risk of offending, violence and exploitation. The role seeks to strengthen relationships with key workers to promote improvements in the health and well‑being of this client group, including health screening, signposting to relevant services, liaison with health colleagues, and case management for those with more complex or urgent needs.
Jobresponsibilities
- Develop and deliver a health screening tool to identify CYPs physical health needs.
- Triage cases to prioritise need and plan of care.
- Signpost to appropriate services as required.
- Act as an advocate for CYPs, providing additional support when necessary.
- Develop a database of all CYPs engaged with YJS and at‑risk adolescent services, capturing outcomes and informing commissioning and needs analysis.
- Jointly work with YJS colleagues to provide an overview of health needs (with CYP consent).
- Collaborate with the Children Looked After (CLA) service for case discussions of CYPs known to both CLA and YJS.
- Work closely with health specialists on joint care of CYPs with both physical and emotional health needs.
- Build relationships with other key stakeholders to ensure children and young people access appropriate services aligned with their physical health needs.
- Complete reports and engage in relevant governance processes across health and the local authority.
- Provide learning and development opportunities to YJS colleagues to enhance understanding of health needs and interventions.
- Contribute to statutory processes (including safeguarding) as required.
- RGN/RN, RCN
- Evidence of relevant Continuous Professional Development
- Mentorship qualification or Practice Assessor
- Nurse prescribing qualification
- Specialist Community Public Health (SCPHN) qualification
- Experience of working with vulnerable families and implementing evidence‑based practice.
- Experience of working in highly emotional/distressing circumstances.
- Experience of participating in the completion of research/audits.
- Experience of completing health assessments.
- Evidence of multi‑disciplinary integrated team working.
- Ability to work flexibly and respond to changing demands both individually and within a team.
- Knowledge of safeguarding children issues.
- Experience in supporting the development of other practitioners.
- Experience of working with the Youth Justice Service (desirable).
- Knowledge of clinical governance, patient safety, and risk management principles.
- Ability to interpret and present complex information clearly to varied audiences.
- Knowledge of safeguarding children and vulnerable adults.
- Understanding of the health needs of this cohort of young people.
- Understanding of the SEND agenda and the impact of Adverse Childhood Experiences.
- Knowledge of the NMC Code of Conduct and scope of professional practice.
- Good working knowledge of data systems (e.g. Systm One) and maintaining electronic patient records.
- Awareness of Camden's public health strategy or local population health needs.
- Knowledge of principles of audit and research.
- Enthusiastic, adaptable, and proactive in problem‑solving.
- Ability to work independently, exercise initiative, and manage competing priorities.
- Culturally sensitive and aware of the needs of diverse communities.

Salary£56,276 to £63,176 a year per annum (pro rata P/T) inc HCAS.
