An exciting opportunity has arisen for a Band 6 nursing post within the established multi-professional team. The successful applicant will need to be RNLD or RGN qualified to support people with complex health needs.

The Community Learning Disability Service offers integrated specialist health and social care for adults with learning disabilities in Tower Hamlets. it oversees a dedicated day activity project tailored for individuals with learning disabilities.

Main duties of the job
  • To use nursing skills in working with people with learning disabilities and autism to maintain, promote and improve health.
  • Be able to assess and support people with learning disability and complex health needs, autism and commonly coexisting conditions like ADHD and mental illnesses.
  • Be able to fully involve the person with a learning disability and at risk of behaviours that challenge.
  • To support people with learning disabilities to develop their full potential by reducing the impact of their impairment, that could limit their social inclusion.
  • To facilitate access to health services through collaboration and partnership with primary and secondary health, and other relevant services.
  • To use a person‑centred approach to promote choice, independence and inclusion.
  • To contribute to shaping local health services in the way they are organised and run to promote an inclusive service for people with learning disabilities.
  • The role involves undertaking assessments, reviews and care planning for NHS Continuing Healthcare Funding in accordance with the National Framework for NHS Continuing Healthcare.
  • Understand the implication of the legal and policy framework underpinning the support of people with a learning Disability and autism.

The post holder is lined managed by the Care Team Manager (Integrated Health & Social Care) - Integrated Multi‑professional Team

Post holder is professionally accountable, to the Clinical Nurse Specialist (Lead Nurse)

Maintain multi professional working relationships within CLDS, Primary and secondary care services and other agencies that support people with learning disabilities in Tower Hamlets

Person Specification Education/ Qualification/ Training
  • RNLD or RMN (Diploma or Degree)
  • NMC 3272 (or equivalent) Supporting and Assessing Learning in Clinical Practice course
  • Community Specialist Practitioner:
    Learning Disability
Experience
  • Minimum of 2-3 years post graduation experience of working as a nurse with people with learning disabilities.
  • Experience of working as part of a team.
  • Experience of supervising staff.
  • Experience of networking with other services and organisations
  • Managing a caseload in a community setting with complex needs
Knowledge
  • Ability to effectively undertake nursing assessments and formulate nursing interventions.
  • Ability to organise and develop new initiatives in care delivery
  • Ability to communicate clearly both verbally and in writing
  • Ability to prioritise, manage and organise workload effectively and efficiently.
  • Ability to provide advice and support on various aspects of Health Care to individuals with learning disabilities, their carers and other professionals.
  • Ability to organise and supervise student nurse placements with the team.
  • Ability to work independently in organising, planning and delivering health care services to people with learning disabilities and their carers
  • Ability to formulate, implement and review health care plans.
  • Ability to respond sensitively to needs of a multi‑cultural community.
  • Ability to carry out research.
  • To have effective interpersonal skills
  • Able to manage electronic diary and have a good knowledge of IT
