About the role

We are delighted to recruit for two permanent Social Worker to join our Charing Cross Hospital Discharge Team.

The 3B Hospital Social Work Team offers a hosted hospital discharge model, providing assessment and discharge support to residents of the 3 Boroughs:
Westminster, Royal Borough of Kensington and Chelsea and London Borough of Hammersmith and Fulham. The Team at Charing Cross works with Imperial Trust and covers both Charing Cross and Hammersmith Hospitals. There is also a Neuro Rehabilitation ward on site CNRU at Charing Cross.

You will be part of a collaborative and effective management team that covers the 3B Hospital Team, including Chelsea and Westminster Hospital in RBKC and St Mary’s Hospital in Paddington, and the Step Down Team in LBHF.

The role will comprise completing care act assessments, facilitating safe discharges and partnership working with our partners; you will hold a case load of cases, some of them complex discharges, supporting the senior practitioner and the Team Manager with facilitating discharges, providing supervision to student social workers, ensuring excellent standards of Social Work practice underpinned by the Care Act 2014 and the Mental Capacity Act 2005, and supporting the team to discharge residents as safely and timely as possible.

This busy and challenging role requires close integrated work with NHS/Health Colleagues both in the Acute Hospital trusts and the Community Health Teams, Community Independence Service, local CCG’s, Continuing Health Care Teams, CLCH and CNWL Home First Teams.

Recognised social work qualification (DipSW, CQSW, CSS or approved equivalent, BA Hons Social Work or other relevant degree in Social Work) and registration with Social Work England (SWE).
