Cardiology & Cardiothoracic Advanced Nurse Practitioner

Here at Imperial Healthcare NHS Trust, we are expanding our cardiac department. We are seeking experienced Band 7 Advanced Nurse Practitioners qualified in Physical assessment and Nurse prescribing.

The successful candidates will be expected to perform across the four pillars of advanced practice. This is a seven day service involving long-day shifts but does not include nights.

This role is critical for patient safety and efficient patient flow within cardiac acute settings. Advanced Nurse Practitioner's provide advanced clinical assessment, prescribing, new admissions, discharge planning, transfers supporting critical care transitions, intensive care unit step-downs to all areas of cardiac services and managing emergencies such as bleeding or cardiac arrest.

This role involves liaising with other referring hospitals for repatriation post treatment or intervention and play a key part in safe discharge home following Occupational Therapy /Physio and Discharge Team input.

Act as a role model and leader for other staff, influencing and developing clinical practice within the service. Educate and support other team members, such as junior colleagues and students, by providing mentorship and clinical supervision.

Critically engage with research and use evidence-based strategies to improve and enhance patient care. Initiate and lead clinical audits to monitor and improve standards of care.

Main duties of the job

Advanced clinical practice embodies the ability to manage clinical care in partnership with patients, families and carers. It includes the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes.

  • Review and prescribe medication in a timely manner ensuring it is within own level of knowledge and competence and within the scope of the Trusts Policy for non-medical prescribing qualification formulating of discharge documentation including medications and onward follow up care.
  • To review urgent patient referrals for cardiac patients within the trust and coordinate the urgent referrals from other NHS organisations. Communicating with the Senior Registers and Pre assessment team to manage and plan the patient in preparation for surgery.

Person Specification Education/Qualifications
  • Registered General Nurse (NMC)
  • Advanced or Intermediate Life Support provider
  • Evidence of accredited post registration level 6 education development in relevant subject (Physical assessment/ clinical reasoning )
  • Evidence of Independent prescribing qualification (or working towards)
  • Master's Degree of /in Advanced Practice (or working towards)
  • Relevant Post basic course
  • Teaching qualification
  • Leadership Qualification
Experience
  • Recent & Significant post registration nursing experience of caring for patients with cardiac disease.
  • Excellent Understanding and experience in recognition of cardiac emergencies.
  • Familiar with national guidelines for CHD content and relevant NICE guidelines
  • Experience of the treating cardiac patient.
  • Database management
Skills/knowledge/ abilities
  • Evidence of developing relevant clinical expertise
  • Evidence of effective interpersonal skills and ability to liaise with MDT teams
  • Venepuncture and Cannulation
  • Involvement in relevant professional networks/group
  • Research experience
