The Clinical Facilitator will lead the delivery of the NCL General Practice Change Management Support Programme at borough and practice levels. They will act as the clinical interface between GP practices, PCNs, federations, and the central programme team, ensuring clinical credibility, alignment to local priorities, and engagement from frontline staff. They will support practices to embed improvements in access, long‑term conditions, and digital transformation, champion quality improvement (QI) approaches, and enable evidence‑based models of care.

• Act as the clinical lead for the borough’s delivery of the Change Management Programme.
• Engage local GPs and clinical teams, ensuring alignment with clinical priorities and contractual requirements.
• Champion the Modern General Practice Model and proactive LTC care delivery.
• Support practices to apply QI methodologies (PDSA, Kotter, Theory of Change) and embed structured improvement processes.
• Use clinical insight to identify barriers, enablers, and opportunities for improvement.
• Collaborate with Business Change Facilitators to design and test interventions that improve access, LTC management and digital tools.
• Provide clinical input to playbooks, improvement frameworks and evaluation tools.
• Lead peer‑to‑peer learning sessions and contribute to borough or NCL‑wide communities of practice.
• Ensure changes to pathways, workflows or processes are safe, evidence‑based and compliant with national and local standards.
• Escalate risks, clinical issues or safety concerns through the agreed governance routes.
• Support measurement of programme metrics (patient experience, access data, LTC indicators) and contribute to reporting.
• Build trusted relationships with practices, PCNs, federations, ICB teams and Lived Experience Representatives.

• Essential:
  Qualified GP, Nurse, Pharmacist or Allied health professional with good standing registration and CPD.
• Significant experience in general practice or PCNs with demonstrated leadership/facilitation in clinical improvement or transformation.
• Understanding of QI and change management methodologies (PDSA, Kotter, Lean).
• Strong communication, facilitation and relationship‑building skills; ability to engage and influence peers.
• Desirable:
  Coaching/mentoring experience, collaboration with voluntary/community sector, digital tools/modern GP model knowledge, formal QI or project management training.
