Care Coordinator
Location: Westminster
Salary: £28,000- £30000
Hours: 40 hours per week
Reports to: Registered Manager / Operations Manager
Job PurposeThe Care Coordinator is responsible for the effective coordination and delivery of high-quality, person-centred care within domiciliary and extra care services. The role ensures service users receive safe, responsive, and well-planned care that promotes independence, dignity, and wellbeing.
Key ResponsibilitiesService Coordination- Coordinate and manage day-to-day care service delivery.
- Develop and maintain staff rotas to ensure adequate care coverage.
- Allocate care workers based on skills, experience, and service user needs.
- Ensure continuity of care wherever possible.
- Conduct service user assessments.
- Develop, review, and update comprehensive care plans.
- Complete and maintain detailed risk assessments.
- Ensure care plans are person-centred and outcome-focused.
- Ensure services meet regulatory standards and organisational policies.
- Maintain accurate and up-to-date documentation and records.
- Monitor care delivery and address any quality concerns promptly.
- Support audits, spot checks, and service reviews.
- Support care staff with guidance and daily coordination.
- Identify training needs and escalate to management where necessary.
- Assist with induction of new staff members.
- Act as the main point of contact for service users and families.
- Liaise with healthcare professionals, social workers, and external agencies.
- Respond to concerns, complaints, and feedback professionally and promptly.
- Oversee safe medication administration practices.
- Ensure health and safety policies are adhered to.
- Coordinate emergency responses and report incidents appropriately.
- Minimum 6 months’ experience in domiciliary or extra care services.
- Experience in writing care plans and risk assessments.
- Strong organisational and time management skills.
- Good understanding of adult social care regulations.
- Excellent communication and interpersonal skills.
- Ability to work under pressure and manage priorities.
- Valid DBS (or willingness to obtain one).
- NVQ Level 3 in Health & Social Care (or working towards).
- Previous experience in a coordination or supervisory role.
