Job overview
The post-holder will be expected to carry out an assessment and formulate a plan of care for service users who are experiencing behavioural and psychological symptoms of dementia or challenging behaviour in the context of mental illness. They will coordinate a defined caseload comprising people with a diagnosis of dementia who are experiencing Behavioural and Psychological symptoms associated with the illness.
This will include developing and evaluating care plans in conjunction with the multi-disciplinary team
The post will have a key role in providing time-limited support to people with dementia, their carers and relatives, and will liaise with colleagues within primary care, community health and social care services, voluntary organisations and the wider mental health of older adults service, as appropriate.

- Croydon, supports people aged 65 and over who have mental Health problems or are living with Dementia in Care Homes.
As a specialist mental health service, we assess people referred to us. We offer specialist Psychosocial Interventions to residents and care staff, working closely to develop person-centered care plans to meet the needs of people living with dementia where behaviour is challenging. We aim to provide the Community "In Reach " program for older people living with dementia in care homes
