Job summary

Integrated Discharge at Imperial College Healthcare NHS Trust has as an exciting opportunity within the Charing Cross Integrated Discharge Team. We are looking for a passionate and dedicated individual who is expert in the area of complex discharge.

Within this role, you will work alongside the existing discharge team lead, to manage the team day to day, manage your own complex case load, be an expert point of contact for complex discharge issues and oversee the running of the discharge lounge.

As well as being a critical and highly valued member of Team Charing Cross, you will also be a part of the wider trust's Integrated Discharge Leadership Team where you will work towards making trust-wide improvements to hospital discharge and flow with lots of opportunity to contribute to or lead on Quality Improvement projects and support colleagues on other sites. Our aim is to improve hospital flow, prevent delays, improve patient experience of hospital discharge and work collaboratively with internal and external stakeholders.

Main duties of the jobTo lead and manage the complex discharge team on a day-to-day basis, as part of the Integrated Discharge Leadership team, providing clinical leadership, support and guidance.

To assume responsibility for a caseload of patients with complex needs in a designated area within the Trust. To work autonomously to ensure that as soon as patients are medically stable, they are transferred to the appropriate discharge to assess pathway.

To take day-to-day responsibility for the leadership, delivery and management of the complex discharge service. Plan and organise this efficiently and effectively with regard to patient management and use of time.

To ensure that all staff within the team have appropriate support for their practice and continuing professional development through team meetings, review, supervision, appraisal, training and development.

To review and update the documentation relating to discharge planning to improve the quality accuracy and consistency of information recorded and to undertake annual documentation audits.

To provide education to clinical and nursing staff regarding the remit of the Discharge team.

The post holder is required to work out of hours (specific rota details held locally within departments).
