St Luke’s Hospice in North West London invites applications for Pall
24 Care Co-ordinators. Based at the Integrated Care Co-ordination Hub in Kenton, you’ll coordinate referrals and support patient care pathways while working with a multidisciplinary team to deliver timely, person-centred care.
This permanent role is 37.5 hours per week (part-time considered) with regular travel to partner sites; night shifts may be required as services develop.
