Health Bridge Direct, the Redbridge GP Federation, is recruiting Clinical Pharmacists to support Primary Care networks across the borough. You will deliver medicines optimisation, safe prescribing, and structured medication reviews within a multidisciplinary team, under supervision of senior clinical pharmacists and a GP supervisor.
The post focuses on polypharmacy management, chronic disease reviews, antimicrobial stewardship, and contributing to NHS-led programmes to improve patient outcomes
