Job overviewJoin our Cardiac, Cardiology and Thoracic Pre-Assessment team and play a vital role in delivering high-quality, patient-centred care to individuals undergoing complex cardiac, cardiology and thoracic procedures. This exciting specialist nursing opportunity offers the chance to work at the forefront of elective and urgent patient pathways, supporting patients from the point of assessment through to surgery or intervention and helping to ensure they receive the safest and most effective care possible.
In this role, you will be responsible for undertaking comprehensive pre-assessment consultations, identifying and managing clinical risks, and optimising patients prior to treatment. You will provide expert advice, education and support to patients and their families, helping them understand their treatment journey and empowering them to make informed decisions about their care. The role requires a holistic approach, combining excellent clinical assessment skills with compassion, empathy and a commitment to delivering an outstanding patient experience.
This position offers a unique opportunity to develop specialist knowledge and expertise within highly specialised services while also contributing to service development, quality improvement projects, audit and research activities.
Main duties of the job- Undertake specialist pre-assessment and optimisation of cardiac, cardiology and thoracic patients.
- Deliver holistic, evidence-based care and support to patients and carers.
- Coordinate patient pathways and advocate for patients throughout their treatment journey.
- Maintain effective communication across services and disciplines.
- Share best practice and contribute to consistency, high standards of clinical care.
- Support and develop junior staff through teaching, mentoring and supervision.
- Complete cardiac and thoracic face-to-face pre-assessments and cardiology telephone assessments.
- Manage and act on Orthus (Virtual Ward) referrals.
- Review and complete Surgical Site Infection (SSI) surveillance.
- Contribute to service redesign and pathway development.
- Participate in policy development and implementation.
- Promote efficient use of clinical resources and equipment.
- Contribute to maintaining a safe and effective clinical environment.
