Complex Care Coordinator Walton-on-Thames Salary: £15 per hour Full-time, Permanent We are looking for an experienced and compassionate Complex Care Coordinator to join our team in Walton-on-Thames. You will be responsible for coordinating complex care packages, supporting care teams, carrying out client visits and reviews, maintaining care plans, and working closely with families and healthcare professionals to ensure high-quality, person-centred care.

About You Experience in complex care, domiciliary care, healthcare or a similar setting. Excellent organisational and communication skills. Confident supporting and coordinating care staff. Knowledge of safeguarding, risk assessments and care planning. Compassionate, reliable and able to work independently. Full UK driving licence and access to your own vehicle is essential. Able to travel between client locations.
