Job Description 3 months contract with a Local Authority

Job Purpose

As an Assessment and Enablement Officer, you will review adult social care packages and support arrangements, helping adults achieve greater independence, wellbeing, and positive outcomes. You will work with people with learning disabilities, autism, mental health needs, physical disabilities, and older adults, ensuring support remains person-centred, strengths-based, and outcome-focused.

Key Duties/Accountabilities
  • Conduct reviews of care and support packages across Adult Social Care services.
  • Assess whether eligible needs are being met and intended outcomes achieved.
  • Carry out strengths-based assessments and reviews.
  • Review risks, needs, wellbeing, and changing circumstances.
  • Promote independence, wellbeing, choice, and control.
  • Work with residents, carers, advocates, providers, and partner agencies.
  • Apply the Care Act 2014, Mental Capacity Act 2005, and safeguarding principles.
  • Complete support planning and risk assessments.
  • Challenge support arrangements where outcomes are not being achieved.
  • Make evidence-based recommendations regarding future support needs.
  • Produce clear review reports and assessment documentation.
  • Identify themes and opportunities for service improvement.
  • Support quality assurance and best practice across Adult Social Care.
Essential Experience Required
  • Experience reviewing Adult Social Care care packages.
Experience working with:
  • Learning Disabilities (LD)
  • Autism
  • Mental Health
  • Physical Disabilities
  • Older Adults
  • Experience completing assessments, reviews, and support plans.
  • Strong knowledge of strengths-based practice.
  • Experience working within Supported Living, Home Care, Residential Care, or Community Services.
  • Experience working with families, advocates, providers, and health professionals.
  • Strong report writing and case recording skills.
  • Ability to make sound professional recommendations based on evidence and outcomes.
Essential Qualifications Required
  • Relevant Adult Social Care experience.
  • Enhanced DBS (Adults Barred List).
  • Right to Work in the UK.
Additional Information to Note
  • Working hours:

    36 hours per week.
  • Hybrid working: 2 office days / 3 remote days.
