ResponsibilitiesAbout the Role:
- Support the service user to connect with activities, support to sustain recovery and reach their goals set out in their aftercare plan e.g. training, employment support.
- Facilitate post-treatment groups across the Bi Borough in community settings, outside of CGL hubs, to support individuals in progressing their recovery journey independently of services.
- Provide service users with information on options available them within the service and in their communities.
- Collaborate with the Partnership Lead to engage partner agencies that service users can access for support following treatment.
- Build positive relationships with aftercare providers to ensure recovery support is available to support post-Rehab early abstinence i.e., Build on Belief (BOB), Fellowship Groups, Aspire2be, SMART
- Work collaboratively with the Service User Inclusion Worker to understand post treatment needs, ensuring the voices and preferences of service users are heard and reflected.
- Working with Recovery Coordinators, Peer Mentors/Volunteers to provide Recovery Check-Ins
- Excellent knowledge of inpatient detox and residential services
- Experience of working within multidisciplinary teams and with fostering positive relationships with partner agencies and external providers
- Experience of working with people at all stages of their treatment journey and an in‑depth knowledge of the importance that groupwork and aftercare support offers
- Excellent written and verbal communication skills and the ability to provide effective advocacy when needed

This role is not eligible for visa sponsorship. Applicants must already have the right to work in the UK at the time of application. For applicants with time‑limited visas, unfortunately, we are unable to support new visa applications or extensions.
Full-time hours at Change Grow Live are 37.5 hours per week. For part‑time roles, the salary and payments will be pro rata based on contracted hours.
Salary Range (pro rata if part time)CGL points 23 to 28 (£28,557.79 - £32,802.41)
