Out of Area Flow Practitioner

Belmont Recruitment are currently working with a Preston based Out of Area Flow Team who are looking for a Band 6 Out of Area Flow Practitioner to join them on a contract basis. The Out of Placements (OAP) team have responsibility for having oversight of all Lancashire and South Cumbria residents who are placed in an out of area bed.

The team works closely with the Bed Hub and OAPs providers, monitoring patients in out of area beds and support with safe and timely discharge. Duties | Responsibilities:
* The post holder will be support the Flow and Capacity Team and the Commissioning and Integration team to provide confident experienced clinical input to the Out of Placements (OAP's
* The successful candidate will act as clinical lead to ensure the patients who are identified for discharge have the most appropriate setting to enhance their recovery in a timely manner.
* The post holder will monitor quality across all Out of Area placements (OAP's) commissioned by the trust.
* The practitioner will be expected to travel via car to inpatient settings outside of Lancashire Care to review service users and discuss flow and capacity with private providers.
* Ability to explain clinical information to patients and their carers or representatives in a way that they are understood. This is a 3–6 month contract with potential extension opportunities. This is a part-time position, 30hrs a week, Monday to Friday 9am to 5pm. Pay rate: £24.00 - £25.00 per hour (depending on experience) We also have other positions within mental health in the area that are waiting to be filled.
