** At Blackheath Brain Injury Rehabilitation Centre, we are specialists in the treatment of patients over the age of 18 with physical and cognitive impairments and/or neuropsychiatric disorders resulting from a brain injury or other neurological condition. We support our patients through assessment, rehabilitation, support and care across two wards.
** What you'll be working:
** Your hours are based on a rota working every other weekend.
* Full-time hours available
* 37.63 hours per week
* Candidates must be able to work day & night shifts
* Shift times (07:30-19:45) & (19:30-07:45)* £27,960.51 (FTE) Per annum, pro rata
** What you'll be doing:
*** Providing a safe, caring, and supportive environment for adults with diverse needs, including mental health conditions, complex care requirements, or those undergoing rehabilitation.
* Supporting individuals with daily routines, including personal care, meal preparation, medication prompts, mobility, and household tasks, in line with their independence and preferences.
* Ensuring the safety, well-being, and dignity of residents by adhering to safeguarding policies and procedures, and reporting any concerns, incidents, or disclosures in accordance with safeguarding protocols.
* Implementing individualised care and support plans tailored to each person’s goals, needs, and outcomes, regularly contributing to reviews and updates.
* Maintaining accurate and timely records of care delivered, daily activities, incidents, and progress, using both written and electronic systems.
* Supporting individuals in managing behaviours that challenge through positive behaviour support strategies, de-escalation techniques, and promoting emotional regulation.
* Encouraging engagement in meaningful activities, therapeutic programmes, community outings, and social interactions that support mental, emotional, and physical well-being.
* Promoting independence, choice, and inclusion in all aspects of daily life, empowering individuals to participate in their care and decision-making.
* Collaborating effectively with colleagues, healthcare professionals, social workers, therapists, and external agencies to ensure consistent, high-quality support.
** What you'll have:
*** A genuine interest in providing person-centred care to adults with a range of needs, including those with mental health conditions, complex care requirements, and those in rehabilitation settings.
* Empathy, patience, and a respectful, non-judgemental approach when supporting vulnerable adults, always promoting dignity, independence, and choice.
* Experience with behaviour management and Positive Behaviour Support (PBS), including the use of de-escalation techniques in a care environment.
