South West London and St George's Mental Health NHS Trust is seeking an Acute Care Pathway Coordinator to maintain ACP operations within bed capacity during OOH periods and provide leadership during working hours. The post holder will coordinate access to Acute, PICU, Older Adult beds, Crisis Hostels, PDU and HBPOS capacity, prioritising based on clinical need and risk.
They will lead suspension of delays through discharge planning and work with senior clinicians to reconstitute capacity,
