Job Overview
Brent Integrated Care Partnership has a number of exciting opportunities available, supporting the borough based partnership to deliver Integrated Neighbourhood Teams in a changing and evolving health and care landscape, as well as running a range of health and care transformation projects across the partnership.
In light of the local ICB restructuring and the selection of CNWL as the Brent “Integrator” function, the Brent ICP Partners are restructuring their team, leading to a number of vacant positions becoming available. We have a number of roles available, hosted by CNWL but working as part of a wider partnership of health and care organisations in Brent. Brent Integrated Care Partnership (Brent ICP) brings together health and care organisations from across the borough and aims to work collaboratively with all the health, care and wellbeing organisations that serve the community of Brent.
MainDuties of the Job
- This role will take on the lead role to tackle Health Inequalities (HI) in Children and Young people (CYP) and the implementation of the Brent Neighbourhood Delivery model.
- To work collaboratively as a member of the Senior Management Team within Brent Neighbourhoods programme and ICP focusing on CYP driving change agendas.
- To lead on developing clinical policy and operational pathways within CYP in accordance with identified Health Inequalities priorities.
- Working with and supporting Cabinet members, lead members and Brent Integrated care partnership to ensure engagement with the Health Inequality project.
- To ensure effective performance management and professional and clinical leadership of the workforce.
- To deliver effective leadership and direction to the Team Managers within their portfolio.
- To ensure services are cost effective and delivered within budget.
- To lead change management in the implementation of service development strategies.
- To create a culture in which innovative ideas and strategies can flourish and provide vision and inspiration to enable staff to deliver care more effectively.

Job Description And Main Responsibilities
The post holder has managerial responsibility for the effective operational running of the community health inequalities team within Brent and will lead on the delivery of a complex portfolio focusing on the needs of Children and young people across Brent.
The post holder will work with the Senior Management Team both within CNWL, the local authority and the Partnership to ensure that the strategic direction for health inequalities for CYP, as defined within the Borough, is effectively implemented at a strategic and planning level and at an operational level.
The post holder is a member of the Senior Management Team and as such has a responsibility to participate in the management of the service and provide sound advice and guidance to the Head of Place (Inequalities and Neighbourhoods), the Managing Director of the ICP and wider Partnership SMT.
