Transfer of Care Lead

This post is a partnership role working across GSTT & colleagues from Southeast London (SEL) discharge services. They will be responsible for the development and delivery of discharge services. The post holder will work in line with agreed objectives reflecting the targets, quality standards, resource controls and constraints and will contribute to delivering the management strategy of the discharge service within the Trusts.

The successful candidate will be a skilled communicator and be able to communicate highly complex and sensitive information at all levels across multiple Organisations. The post holder will be able foster strong relationships at senior levels with key strategic and delivery partners, acrosstheNHSandwiderhealth and caresectorandexperta creating co-operationandcollaborative inworking across  line withtheseresponsibilities, thepostholderwill overseeandensurethe development and coordination ofstrongdeliveryplansacross the discharge/transfer of care Services.

• Strategic and Operational Development:lead the strategic and operational direction of the discharge/transfer of care services across GSTT & SEL and effectively mainstream strategic priorities into commissioning and provider business across the healtheconomy.
• People management: lead and develop high performing local delivery team, and line manage ateam.
• Partnership working: effective relationship management and engagement with senior clinical and non-clinical stakeholder; developing strong and credible network relationships with partner Organisations within SEL.
• Design and lead delivery of significant transformational change regarding how we manage transfers of care/discharge, to meet trust strategic and operational objectives across the Trusts leading to measurable improvement in efficiency, safety and quality of care for patients.
• Lead effective implementation of the programme plan with appropriate monitoring arrangements inplace.
• To implement a comprehensive programme dashboard effectively utilising existing data for the measurement of performance and modelling of outcomes, where possible working with IT colleagues in using electronic media to doso.,

• Lead on horizon scanning activities and identify local, national and international best practice to inform transfer of care/discharge services and pathways across Organisations andwhereappropriateensurethat it isadoptedwithin the portfolio
• Extensive experience in synthesis and analysis of highly complex data from national and local sources and translating data to the Board
• Use complex data and skilled judgement in interactions withstakeholderstoshapethe design of the portfolio
• Promote innovation and research across transfer of care/discharge services, ensuring any research is translated into best practice
