We are looking for an enthusiastic and reliable Practice Nurse to support our practice team in delivering primary care service to our diverse patient population.
You will be working with a varied patient demographic and an experienced Multi-disciplinary team.
Work Schedule Requirement: The 16 hours must be worked over 2 days. Preferred day combinations are:
- Monday and Wednesday
- Wednesday and Friday
- Monday and Friday
The ideal candidate will have experience in the core competencies of practice nursing including wound care, child immunisation, independent and proactive chronic disease management (in particular diabetes) and cervical cytology. In addition, experience of management responsibilities would be beneficial, as there will be scope for the successful applicant to take on additional individual organisational and management responsibilities.

Job responsibilitiesJob summary:
Helping the practice develop and deliver specialist primary nurse led services including chronic disease management. Make professionally autonomous decisions and provide safe, evidence based, cost effective, individualised patient care
- Providing assessment, screening and treatment services and health education advice
- Cytology
- Wound care
- Travel Clinic
- Asthma/Peak Flow Readings
- Routine immunisations
- Childhood immunisations
- Requesting pathology tests, for example urine culture, swabs
- Diabetic reviews
- Maintain general tidiness and cleanliness of treatment rooms
- Maintain stock of waiting room leaflets as instructed by partners and Practice Manager
- Ordering of vaccinations to maintain stock levels
- Attend and participate in any Practice meetings when required.
- Following appropriate training, undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc. and ensure the cleanliness of any equipment used. Perform any investigatory procedures requested by the GPs.
- Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate.
- Ensure accurate completion of all necessary documentation associated with health care and registration with the practice.
- Maintain a notice board in the waiting area designated for patient health care education.
- Qualified Nurse
- Qualified in management of diabetes
- Experience working within a General Practice as a Practice Nurse
- Chronic disease management
