We are looking for experienced B5 nurses to join our Neighbourhood Nursing teams across Lambeth and Southwark areas with extensive experience in a community setting, possessing the skills outlined in the job specification.
Main duties of the jobThe post-holder will be a member of the Neighbourhood Nursing (District Nursing) Team and will participate in the clinical case management of patients who have a wide variety of complex, high-risk and supported/self-care conditions. Working directly with the Team Leader, the post-holder will help ensure the provision of a high standard of nursing care and health promotion. Care will be provided in a range of settings including patients' homes, residential homes, nurse‑led clinics and GP practices.
- Holistic assessment, planning, implementation and evaluation of a plan of care for patients with long‑term conditions, tissue viability, continence, palliative care, and promoting/facilitating patient independence.
- Assists in the delivery of National Service Frameworks.
- Providing public health and health promotion programmes to improve health and reduce inequalities.
- Maintaining contemporaneous written and computerised records.
- Registered Nurse (General Adult)
- Degree level education or equivalent.
- Post‑registration course in wound management, diabetes, leg ulcer care, continence care and assessment.
- Extensive experience in caring for people with long term conditions in a community or primary health‑care setting.
- Experience in caring for people with long term conditions.
- Acute hospital setting experience.
- Able to assess, plan, implement & evaluate programmes of care.
- Demonstrate evidence‑based care and decision‑making responsive to the needs of clients and carers, negotiated in partnership.
- Competent in basic life support.
- Operate as an autonomous practitioner, being responsible and accountable for choices made.
- Work flexibly and respond to changing demands in workload.
- Communicate effectively at all levels within the organisation, with patients, relatives, carers and other agencies.
- Write detailed reports, maintain contemporaneous patient records and documents with fluency and accuracy.
- Skilled in providing hands‑on direct patient care, including safe handling of patients and equipment.
