Advanced Nurse Practitioner GP practice-2 days/week
- Total 16 Hours
The post-holder will work as an autonomous, accountable advanced practitioner, providing a complete episode of care to patients presenting with undifferentiated and undiagnosed problems. Working within their scope of practice and as an independent prescriber, the ANP will assess, diagnose, treat, prescribe for, refer and signpost patients of all ages across acute, minor illness and long-term condition presentations, both in surgery and during home visits to housebound patients
Main duties of the jobClinical practice
Provide first-contact clinical assessment for patients presenting with undifferentiated, undiagnosed problems- taking history, examining, investigating, diagnosing and managing the episodeof care.
Manage same-day, acute andminor illness presentations across all age groups, including telephone andonline triage where required.
Independently prescribe, reviewand de-prescribe medication within own competence and in line with NICEguidance, the BNF, MHRA safety alerts and the practice formulary
Proactively manage patients with long-term conditions e.g. diabetes, asthma, COPD, hypertension,cardiovascular disease, including structured reviews, treatment optimisation and care planning.
Undertake home visits tohousebound and frail patients, exercising sound clinical judgement on what canbe safely managed in the community.
Recognise, assess and managedeteriorating, acutely unwell and emergency presentations, escalatingappropriately to the duty GP or emergency services.
Request, interpret and act oninvestigations and results, and arrange onward referral to secondary care andother services as clinically indicated.
Work within own clinicalcompetence, seeking advice from or referring to a GP or appropriate colleaguewhere a presentation falls outside scope of practice.
About usHounslow Family Practice is a GP practice serving approximately 6200 patients in Hounslow. We are looking for an experienced and autonomous Advanced Nurse Practitioner to joinour multidisciplinary team for two days each week (Mondays and Thursdays).
Job responsibilitiesAlongside Clinical practice we expect competence in the below:
Safeguarding, safety & qualityRecognise and act onsafeguarding concerns for children and adults at risk, in line with practice and local safeguarding partnership policies and statutory guidance.
Maintain accurate,contemporaneous, legible clinical records and appropriate coding within the clinical system.
Contribute to significant eventanalysis, complaints responses and learning, and support the practice inmeeting its Duty of Candour
Participate in clinical audit,quality improvement and medicines optimisation activity.
Adhere to infection prevention and control standards at all times.
Professional & team workingMaintain registration with the Nursing and Midwifery Council (NMC) and revalidate in line with NMCrequirements.
Practise within the NMC Code
-legally, safely and ethically - and within agreed scope of practice and protocols.
Maintain and develop owncompetence through continuing professional development, appraisal andreflective practice.
Support, supervise and act as aclinical resource for practice nurses, HCAs and wider team members as appropriate.
Contribute to the development of practice protocols, patient group directions and clinical pathways.
Work collaboratively with thewider primary care network PCN and community teams.
Person Specification Experience- Experience of working in primary care
- Experience of working in a GP practice
- Experience of working within a nursing team
- Experience of dealing with vulnerable patients
- Registered Nurse with current NMC registration
- Master's-level qualification in Advanced Clinical Practice, or equivalent demonstrable Master's-level competence
- Relevant post-registration clinical qualifications, e.g. minor illness, long-term conditions, physical assessment
- Evidence of ongoing CPD and NMC revalidation
- Mentorship / teaching qualification (D)
