The clinical and operational lead for Hospital at Home will incorporate the dimensions of expert clinical practice, education, leadership, management and research.

• Lead, negotiate, supervise and manage caseloads of patients within nurse-led clinics both on and off site using advanced clinical assessment skills and clinical judgement to take detailed history, physically assess, make differential diagnoses, order tests or investigations, prescribe medication, evaluate and review treatment plans.
• Provide professional, clinical and managerial leadership to nursing and related staff, ensuring provision of a high‑quality, responsive nursing service.
• Liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of specialised patient care across professional boundaries within a virtual ward setting.
• Autonomously deliver high‑level clinical care appropriate to the needs of the patient group, based on research and evidence, and enhancing patient care quality.
• Plan, coordinate, deliver and evaluate nurse‑led services, ensuring excellent standards of nursing care.
• Line‑manage clinical nurse specialists within speciality and across boundaries.
• Lead and support development/implementation of nursing programmes for individual patients and the client group as a whole.
• Facilitate the planning, delivery and evaluation of care pathways for a designated group of patients.
• Contribute to training and education within the specialist area.
• Promote an innovative and progressive attitude to continual improvement of patient care through reviewing the evidence base and embedding this in practice, engaging in research activities and quality improvement work.
• Act as a visible role model and expert practitioner.
• Collaborate and contribute to quality management related to nursing and clinical area, ensuring alignment with national policy and local agenda.
• Ensure safeguarding procedures are followed and remain knowledgeable about safeguarding triggers and risks.
• Assume professional and legal accountability for clinical and professional activities, recognising limits of knowledge and seeking medical support when required.
• Ensure core, specialist and advanced nursing practice developments meet service needs, link research and evidence‑based practice to patient safety agendas.
• Investigate complaints and untoward incidents, implementing recommendations and related education/training programmes/action plans.
