Dialysis Nurse, Clinical Nurse Specialist
We are seeking a motivated and enthusiastic Band 6 Diabetes Specialist Nurse to join the North Middlesex Diabetes Team and support the delivery of high‑quality, patient‑centred diabetes care across the Trust.
The post holder will work across inpatient and outpatient settings, supporting the assessment, management and education of adults with diabetes, including those with complex needs. Working alongside senior Diabetes Specialist Nurses and the wider multidisciplinary team, you will contribute to safe and effective diabetes management, helping to improve glycaemic control and reduce diabetes‑related complications.
You will provide specialist nursing input to ward teams, supporting insulin management and the recognition and management of hypoglycaemia and hyperglycaemia, in line with local and national guidance. Patient education and staff training are key aspects of the role, promoting best practice in insulin safety, sick day rules and diabetes self‑management.
The role involves close collaboration with diabetes consultants, dietitians, podiatrists, pharmacists, and community and primary care colleagues, as well as participation in audit, service development and quality improvement activities.

Main duties of the job- Deliver expert, evidence‑based clinical care for patients with complex diabetes needs in inpatient and outpatient settings.
- Assess, prioritise and manage referrals, supporting safe insulin use, optimisation of glycaemic control and early discharge planning.
- Provide specialist advice to patients, carers and staff, including insulin initiation, dose adjustment and prevention of acute complications.
- Lead and contribute to nurse‑led and MDT diabetes clinics.
- Act as a clinical role model, supporting high‑quality, patient‑centred care.
- Deliver education and training to patients, families and multidisciplinary staff.
- Support and lead service development initiatives, audits and implementation of evidence‑based guidelines.
- Work collaboratively within the multidisciplinary team and across organisational boundaries to ensure seamless care.
- Maintain professional development in line with NMC requirements, Trust policies and national standards.
- Act up for the senior DSN as required and organise own caseload effectively.

- Experience of working in other areas relating to speciality
- Delivers high standard of patient focused nursing care.
- Excellent written and verbal communication skills.
- Expert up‑to‑date clinical knowledge in own speciality including relevant NICE guidelines and other national strategies relating to clinical speciality.
- Has gained mentorship course e.g. ENB 998 or equivalent.
- Kind and empathetic approach to both patients and staff.
- Positive and confident nature.
