This role is for a practice nurse based at Northern Medical Centre. It is an exciting opportunity to work within an innovative and forward‑facing clinical model for the changing times of general practice. As the practice nurse, you will be a member of the clinical team and help deliver nursing care for long‑term conditions within a large multi‑professional team.
Main duties- Work in a multidisciplinary primary care team across Islington General Practice Federation.
- Participate fully in clinical and team meetings as a member of the wider multidisciplinary team.
- Lead on the replenishment of stock in the practice.
- Deliver treatment room nursing duties and keep up to date with the following competencies:
- Smear taking
- Immunisations (including large‑scale immunisation clinics)
- Basic observations
- Provide routine nursing care to patients through up‑to‑date evidence‑based practices.
- Conduct long‑term condition reviews using evidence‑based practice via face‑to‑face, telephone, and text.
- Refer to secondary/specialist care as needed and within scope.
- Administer medication and immunisations as per Patient Group Directions and keep updated with new directions.
- Guide patients in the use of prescribed and over‑the‑counter medications, side effects and dosages; support care navigation, signposting and streamline information.
- Coordinate the provision of travel medicine services.
- Request pathology services and process results as required.
- Maintain accurate clinical records in line with legislation and the NMC Code; use Read codes effectively.
- Ensure compliance with practice policy, Infection Prevention & Control and infection control protocols.
- Prioritise health issues and intervene appropriately; support the team in clinical emergencies and keep Basic Life‑Saving skills up to date.
- Recognise and refer patients with mental health needs; support safeguarding matters in line with policy.
- Maintain a record of vaccines, storage conditions and cold chain; ensure stock is replenished and up to date.
- Commit to learning new IT systems and innovations in patient care; manage tasks efficiently.
- Respect colleagues regardless of role title.

Qualifications- NMC Registered.
- General Practice Nursing Certificate.
- Clear enhanced DBS check.
- Evidence of CPD.
- Minor surgery skills.
- Mentor/teaching qualification.
- Experience working in UK general practice as a nurse.
- Experience with vulnerable groups.
- Clinical experience of 1+ years.
- Experience using EMIS Web, Docman, Accurx.
- Experience with clinical triage.
- Experience working with deprived populations.
- Experience using telephone interpreter.
- Delivery of clinical audit.
- QOF management.
- IT fluency and troubleshooting.
- Use of Teams for policy access and communication.

This post requires a Disclosure and Barring Service check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
£38,000 to £45,000 a year, depending on experience.
