Hospital @ Nurse

Are you an acute children’s nurse looking to branch out into community nursing? Or a Children’s Community nurse looking for more acute focused nursing care at home?

The Royal London Children’s Hospital is recruiting a band 6 children’s nurse to cover maternity leave and support delivery of the innovative hospital @ home service commissioned by Tower Hamlets CCG. The team visits acutely unwell children in the community to offer treatment preventing them from attending hospital and/or staying in longer than needed. The H@H team forms part of the Pan London H@H forum, providing a cohesive and high-quality service to children in Tower Hamlets.

We are looking for experienced and highly skilled children’s nurses who can work autonomously to care for acutely unwell children  successful candidate will have excellent team-working and decision-making skills and put patients first.

• Facilitate efficient and effective patient flow from attendance at the hospital to referral home, helping to ensure we meet the 4-hour wait target in the emergency department.
• Perform highly skilled acute nursing care and assessment for children at home.
• Work closely with ward managers, paediatricians, matrons and the flow team on a daily basis to expedite referrals home.
• Act as an excellent role model and leader for all staff.
• Participate in the development of new pathways of care.
• Support the evaluation of the service regularly and adapt as necessary.
