GP at hand is one of the largest digitally enabled NHS General Practices, providing high‑quality patient centred care through a combination of innovative digital services and face‑to‑face clinical care.
We are seeking enthusiastic and motivated Practice Nurses to join our existing Nursing team, with full and part time opportunities available.
The successful candidate will deliver a broad range of nursing services across our London clinic sites.
This role offers a varied and dynamic workload, combining in‑person clinical appointments, remote consultations, and desktop clinical reviews. The post holder will work collaboratively with GPs, Advanced Clinical Practitioners, Practice Nurses, Nurse Associates, Healthcare Assistants, Phlebotomists, Pharmacists, and other members of the multidisciplinary team to deliver safe, effective, equitable, and accessible care.
We operate from five conveniently located sites across Central London (Canary Wharf, Victoria, Euston, Fulham and Wimbledon), which are open from 8:00am to 8:00pm. Successful candidates should be able to work a range of shifts throughout these hours to support service delivery.
WHAT YOU’LL WORK ON Clinical Practice- Deliver evidence‑based nursing care within the scope of professional practice.
- Undertake a range of face‑to‑face nursing clinics, including:
- Diabetes
- Hypertension
- Cardiovascular disease
- Cervical screening
- NHS health checks
- Child and adult immunisations
- Travel health assessments and vaccinations
- Wound care, dressings and removal of clips / sutures
- Long term disease management clinics, including:
- Venepuncture
- Conduct remote consultations where clinically appropriate.
- Complete desktop clinical reviews, including:
- Long term condition management
- Recall management
- Review of investigations
- QOF related reviews
- Identify safeguarding concerns and Escalate appropriately inline with practice policies.
- Maintain accurate, contemporaneous records using electronic patient record systems.
- Undertake structured annual reviews and contribute to achievement of QOF indicators.
- Support proactive population health management initiatives.
- Promote healthy lifestyle choices and preventative healthcare.
- Provide smoking cessation advice, weight management support, and other health promotion interventions.
- Support national and local screening programmes.
- Adapt communication styles to deliver high‑quality care across face‑to‑face, telephone, video, and asynchronous clinical interactions.
- Contribute to the development and continuous improvement of digital nursing pathways.
- Support patients in accessing and navigating digital healthcare services.
- Work within the Nursing and Midwifery Council (NMC) Code of Professional Conduct.
- Participate in clinical audit, quality improvement projects, and service development initiatives.
- Maintain compliance with infection prevention and control (IPC) standards.
- Report incidents, significant events, and near misses in accordance with practice procedures.
- Contribute to maintaining CQC compliance and high standards of clinical governance.
- Participate in team meetings, clinical supervision, and continuing professional development activities.
- Ensure clinic rooms are adequately maintained, including stocking and rotating items as required.
- On occasion there may be a requirement to undertake home visits or be asked to move location to cover absence.
- Registered Nurse with current NMC registration.
- General Practice Nurse Qualification or accredited long term condition modules + ability to demonstrate individual competencies.
- Experience delivering the national immunisation programme to identified population groups.
- Cervical cytology training and competence.
- A willingness to adapt working practice to meet changing practice and service development.
- Flexibility of working across sites if required.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a varied workload.
- Confident in delivering care through digital and remote consultation methods.
- Understanding of QOF and primary care quality standards.
