Childrens Community Nurse - band 6. The post‑holder will provide high‑quality, specialist community nursing care to children and young people with acute, chronic, complex and palliative health needs. Care will be delivered in homes, schools, clinics, and other community settings, ensuring children can access education, daily activities & safe care closer to home.

• Assess, plan, implement and evaluate programmes of care for children requiring nursing support at home or in community settings.
• Provide specialist nursing interventions including gastrostomy care, accessing central lines, suctioning, medication administration, and long‑term condition management.
• Support inclusion of children with health needs in nurseries, schools, and community activities.
• Contribute to palliative and end‑of‑life care, including participation in the on‑call rota.
• Follow all safeguarding policies and procedures, ensuring concerns are escalated appropriately.
• Promote seamless care between hospital and community services.
• Promote effective teamwork and support colleagues.

• RN Child/RSCN
• Mentorship course or equivalent
• Current NMC registration – child branch
• Evidence of recent Continuous Professional Development
• Evidence of degree level study or equivalent experience
• Specialist clinical courses relevant to community children’s nursing

• Post‑registration experience delivering CYP nursing care across community, primary care or acute settings.
• Experience delivering care to CYP with complex health needs.
• Understanding of community nursing practice including lone working, risk assessment and MDT collaboration.

• Strong clinical skills including gastrostomy care, central line care, suctioning, medication administration and emergency response.
• Effective time management and prioritisation.
• Ability to work autonomously and within MDTs.
• Understanding of safeguarding responsibilities.
