Job overview

We are seeking an experienced and motivated Diabetes (including Endocrinology service) Specialist Nurse to deliver expert, evidence-based care across our Integrated Diabetes services.

Main duties of the job

Clinical Care & Support

  • Provide expert assessment, advice and treatment for people with diabetes and endocrine conditions across hospital, community and outpatient settings.
  • Manage your own patient caseload, delivering individualized care plans to promote optimal health outcomes.
  • Offer ongoing education and support to patients and families to enhance self‑management and quality of life.

Education & Training

  • Teach and train patients, carers and healthcare staff, fostering confidence in managing complex conditions.
  • Develop educational resources and programs tailored to different learning needs and clinical settings.

Collaboration & Integrated Care

  • Work closely with multidisciplinary teams—including doctors, dietitians and pharmacists—to ensure coordinated, holistic care.
  • Act as a key point of contact for complex cases, ensuring effective communication and continuity of care.

Service Development & Quality Improvement

  • Contribute to service development initiatives to enhance care delivery and patient experience.
  • Conduct audits, collect data and apply research findings to improve standards and clinical outcomes.

Leadership & Professional Development

  • Support team growth by sharing expertise, mentoring colleagues, and promoting best practices.
  • Take an active role in clinical governance, policy development and innovation in care pathways.

Detailed job description and main responsibilities
  • Coordinate specialty care within the Trust/CCG, including outreach clinics.
  • Act as an essential member, point of contact and resource for the multidisciplinary team, patients and carers.
  • Carry out diagnostic/therapeutic interventions as an independent practitioner, interpret or write reports.
  • Perform complex therapeutic/investigational procedures under indirect supervision of the Consultant, ensuring safe practice.
  • Demonstrate ability to develop new skills and provide a nurse‑led service.
  • Provide ongoing follow‑up care and support (telephone support, nurse‑led clinics, outreach clinics).
  • Develop the service in a structured interdisciplinary way, initiating ongoing change.
  • Develop interdisciplinary clinical management protocols and work within them.
  • Autonomously or in collaboration manage care of patients within agreed protocols.
  • Provide a supportive service to patients and carers, sharing specialist knowledge.
  • Provide and maintain a support and advisory service throughout the disease process.
  • Maintain accurate and legible records of all clinical and legal documentation.
  • Assess, develop, implement and evaluate programmes of care within acute and community settings.
  • Control patient admission via direct referral with consultant guidance.
  • Participate in the re‑formulation of the nursing aspects of the Divisional Business Plan.
  • Investigate nursing aspects of incidents/complaints and initiate corrective action when required.
Person specification Qualifications and Training
  • Registered Nurse Level 1 register with NMC.
  • 1st Level Degree or equivalent.
  • Teaching and assessing qualification.
  • Diabetes specialist course.
  • Accredited qualification in insulin therapy.
Experience
  • Clinical experience in diabetes in a specialist role.
  • Ability to work autonomously.
  • Good time management and prioritisation skills.
