Adult Community Nursing (ACN) Service – Band 5 Community Staff Nurse
We are looking for an enthusiastic Band 5 Community Staff Nurse to join the Clinical Assessment Team (CAT) on a fixed‑term 12‑month contract within the ACN Single Point of Access.
Responsibilities- Undertake holistic initial assessments of individuals referred to the ACN service.
- Use clinical skills and knowledge to implement care plans, promote independence and self‑care, and manage short‑term care interventions.
- Liaise with ACN neighbourhood teams, GPs, social services, therapists, care agencies and other multidisciplinary teams.
- Work under the guidance of the team’s Nurse Practitioners and Senior Nurse Practitioner.
