To promote expert, individualised patient centric clinical care for clients with compromised skin integrity and pressure ulcers by using evidence based practice for the population of Croydon Health Services (CHS). The pressure ulcer nurse will assess and plan care for patients referred to the service and work in conjunction with the tissue viability team.
The role will include the provision of care and supervision of the delivery of this care by clinical staff.
The post holder will offer education and support to patients, carers and other health care professionals.
The pressure ulcer nurse will work closely with other health care professionals to promote a high standard of care in the prevention and management of pressure ulcers ensuring the pressure ulcer policy & pathway are followed. This is in line with local and national guidelines and best practice in the aim to reduce all grades of pressure ulceration.

This includes the provision of well‑coordinated care and support to promote independence in the most appropriate setting.
We require a full time Band 6 District Nurse/Case holder to work within our new Integrated Care Teams to provide efficient and effective nursing care to patients in their own homes. Excellent clinical, leadership and communication skills are essential along with the ability to work in an innovative way as part of the new Integrated Community Networks. The ideal candidate must be a Registered Nurse with a Specialist Practice Qualification in District Nursing or have experience working as part of the Community Nursing Team.
All candidates must have a clean full UK driving licence and the use of a car for work purposes.
- Registered General Nurse-Adult.
- Specialist Qualification in area of Practice or significant experience in nursing adults in the community.
- Willingness to undertake further training relevant to this post.
- Nurse Prescribing or willingness to undertake.
- Live registration as a sign off mentor.
- Understanding the role and responsibilities of a Caseload Holder in relation to patients and colleagues.
- Must have knowledge and understanding of Health and Safety responsibilities.
- Understanding of the need for confidentiality in relation to work activities.
- Understanding of Equality and Diversity in relation to work activities.
- Understanding of the principles of promoting independence.
- Awareness of own level of knowledge and competency and of when to seek guidance from senior colleagues.
- Awareness of current issues affecting Community care and the NHS.
- Knowledge of the principles of reflective practice.
- Demonstrate knowledge of management and leadership strategies and styles.
- A minimum of 2 years nursing experience at Band 5 in the Community.
- Experience as a Mentor for Student Nurses.
- Experience of caseload management.
- Experience as a sign off Mentor for Student Nurses.
- Experience of financial and human resources issues.
- Ability to assess, plan, implement and evaluate nursing care for patients in their own home.
- Demonstrate a high level of clinical competence.
- Understand the change management process.
- Must be able to demonstrate clear verbal communication skills with patients, carers and staff at all levels including feeding back information.
- Ability to prioritise own and teams workload and organise time effectively.
- Ability to work without direct supervision, e.g. when in patients home and use initiative.
