The Mental Health Crisis Nurse Practitioner (Band
6) is an established clinical post within the City and Hackney Home Treatment Team, operating as part of the wider Crisis Pathway framework. The post holder will deliver urgent community crisis assessments, working within a multidisciplinary team to provide person‑centred care in the least restrictive environment.

The post holder will carry out face‑to‑face mental health and risk assessments, agree crisis safety care plans with service users, carers, and families, and deliver psychosocial interventions within the Recovery Model to support self‑management and recovery.

The role requires liaison with Primary Care, Community Mental Health Teams, Social Services, In‑Patient services, the Voluntary Sector, and Criminal Justice. The post holder will contribute to shift coordination, maintain accurate clinical records and statistical data in line with Trust and NMC requirements, and ensure compliance with safeguarding policies for adults and children.

The role includes supervising and mentoring junior colleagues and student nurses, contributing to quality improvement, and maintaining professional development in line with NMC requirements.

Cover may be required at short notice at Psychiatric Liaison at Homerton Hospital or the Crisis Assessment Centre at the Raybould Centre. The service operates 08:15–21:00, seven days a week, and the post holder will work flexibly to meet the Trust's 24‑hour crisis assessment KPI target.

The post holder will act as crisis coordinator, managing referrals and liaising with Primary Care, Community Mental Health Teams, Social Services, In‑Patient services, the Voluntary Sector, and Criminal Justice to ensure continuity of care.

The post holder will maintain accurate clinical records and documentation in line with Trust and NMC policies, submit statistical data as required, and produce clear clinical reports using Trust IT systems.

The post holder will contribute to shift coordination, respond to emergency referrals within agreed time frames, and ensure compliance with safeguarding policies. The role includes supervising junior colleagues and student nurses, contributing to quality improvement, and maintaining professional development. Using psychosocial interventions and the Recovery Model, they will actively support individuals to self‑manage their symptoms and engage with relevant community resources including the Recovery College.
