The opportunity has arisen for a full-time band 7 Diabetes Specialist Nurse to join a project to provide care to people in Greenwich.

This post focusses on reviewing housebound patients who are under the care of district nursing and require insulin therapy, using Continuous Glucose Monitoring (CGM). The post holder will lead this initiative to optimise diabetes management for this patient group, improve safety and outcomes, supporting the integration of CGM into community-based care. The post holder will provide support and education for housebound patients, their carers and families to enable then to self-administer insulin

We are seeking a motivated, enthusiastic individual who is committed to developing and improving diabetes services for housebound individuals. The service will be delivered in a variety of community locations including peoples own homes, therefore travelling in the borough independently is expected.

YouMUSThold a non-medical prescriber qualification. As a non-medical prescriber with experience in titrating insulin, applicants must be able to work on their own as well as being part of a team. You will be involved in educating staff, patients, their family or carers and other members of the multi-disciplinary team. The post holder will also be supported by the Diabetes Team lead.

The post holder will support the ongoing work to shape the future delivery of diabetes care across Greenwich. The post holder will be responsible for reviewing housebound patients who are under the care of the District Nursing Service and require insulin therapy, with the use of Continuous Glucose Monitoring (CGM) technology. The post holder will lead the initiative to optimise diabetes management for this patient group, improve safety and outcomes, and support the integration of CGM into community-based care.

The post holder will provide sound clinical leadership for service improvement, monitoring and improving standards of care in accordance with National diabetes guidance. They will provide specialised expertise in practice, facilitation and management of change through collaboration, providing vision, education, research and practice development.

The post holder will offer patients, relatives, carers and community healthcare professionals support and advice by providing education programmes tailored to suit needs and will provide specialist expertise and education in diabetes care to GPs, District Nurses, patients, relatives, carers and other health care professionals and will support junior staff in doing so.
