• Community Nursing Care: Deliver specialist nursing care to children and young people within community settings.
• Assessment and Planning: Assess, plan, implement, and evaluate care using a family-centred approach.
• Referral Management: Triage and assess new referrals, prioritising urgent cases appropriately.
• Discharge Support: Facilitate safe and effective hospital discharges into the community.
• Equipment Provision: Assess and arrange equipment and supplies to meet individual care requirements.

• Nursing Registration: Registered Children’s Nurse (RSCN/RN Child) with current NMC registration.
• Experience: Previous experience within children’s nursing is essential.
