Practice Nurse

Job summary

We are looking for an energetic, motivated and enthusiastic Practice Nurse to join our friendly and supportive nursing team which consists of an ANP, 3 Nurses and 3 HCAs.

The post-holder will be proficient in all aspects of general practice nurse duties, and able to deliver high-quality, evidence-based nursing care to our patients, working autonomously within their scope of practice.

You will play a key role in managing long term conditions, delivering immunisations, supporting public health programmes, and promoting health and wellbeing across our local community. • Regular appraisal and clinical supervision
• 24 hours ideally over 3 days to include a Tuesday

• Deliver routine nursing care including wound care, ECGs, and health checks
• Lead and support the management of long-term conditions e.g. diabetes, COPD, asthma
• Deliver immunisations, vaccinations and cervical screening programmes
• Provide health promotion and prevention advice to patients
• Support infection prevention and control IPC standards
• Maintain accurate clinical records using appropriate coding systems
• Participate in clinical audits, quality improvement, and service development
• Work collaboratively within the multidisciplinary team MDT and wider PCN
• Recognise clinical boundaries and refer appropriately to GPs or other professionals
• Support the delivery of QOF targets and enhanced services

We are a large, well-established, patient-centred GP practice that serves a diverse population, which includes managing the medical centre at the University of Roehampton. We are an active Student Nurse, Undergraduate and Postgraduate GP training practice

• Registered Nurse with the Nursing and Midwifery Council NMC
• Competence in core nursing skills e.g. immunisations, wound care, cervical screening
• Ability to work independently and within a team
• Strong communication and interpersonal skills
• Experience working as a Practice Nurse
• Experience in management of long term conditions
• Experience in quality improvement or audits
• Knowledge of QOF and NHS primary care systems
• Understanding of safeguarding, clinical governance, and evidence-based practice
• IT proficiency and ability to maintain accurate clinical records
