Job Summary
Make a Lasting Impact as a Social Worker delivering high‑quality, strengths‑based assessments, interventions and care planning to adults with learning disabilities and autism in Lewisham.
Responsibilities- Deliver person‑centred support empowering individuals and promoting independence.
- Lead comprehensive assessments, care planning and reviews in line with the Care Act 2014 and other relevant legislation.
- Collaborate closely with individuals, families, carers and multi‑agency professionals to ensure holistic and effective support.
- Navigate and apply the Continuing Health Care (CHC) framework, safeguarding protocols, and Mental Capacity Act processes.
- Stay informed about local services and resources to ensure the best outcomes for those you support.
- Qualified Social Worker registered with Social Work England and passed AYSE.
- Strong working knowledge of the adult social care legal framework, including the Care Act, Mental Capacity Act, and safeguarding legislation.
- Experience working with adults with learning disabilities and a commitment to promoting their rights, choices, and wellbeing.
- Excellent communication, assessment, and crisis intervention skills.
- Collaborative mindset and ability to work effectively within multidisciplinary teams.

15 minutes south of London Bridge, based in Catford SE6. Hybrid working approach as determined by service requirements.
Salary£45,852 – £47,925 per annum (SCP 31‑33).
Special Conditions- Qualified Social Workers registered with Social Work England, must have passed AYSE.
- This job is subject to Enhanced Disclosure application to the Disclosure and Barring Service.
- Qualified Social Worker BA /DipSW/CQSW or equivalent and completion of ASYE programme.
- Adherence to standards set out in the Job Description/Person Specification.
