Qualified Social Worker, Continuing Healthcare Team Pay rate £30.66 per hour Contract role Vitalis are recruiting for an experienced Social Worker to work in a Continuing Healthcare Team in Blackpool Council.

We require an experienced Social Worker to cover 37 hours per week.

Social work post that is part of our Continuing Health Care (CHC) Team. The successful candidate will join a well-established and multi-disciplinary team.

The Social Work role in this area is varied and supports our health colleagues with a wide range of tasks. From monitoring long term CHC provision, to the completion of joint Reviews, Safeguarding, Undertaking assessments in line with the Care Act 2014, Decision Support Tool Meetings and Court of Protection work.

You will need to be passionate about supporting our community in Blackpool, be committed to your role as a Social Worker and keen to grow and develop in line with Social Work England protocols.

This is a hybrid role and you will be required to attend the office 3 days per week - staff parking is available at reduced rates.
