Pay rate: £30–£32 per hour (depending on experience). Position type:
Full-time (35 hours per week, Monday–Friday). Working pattern:
Office-based five days per week during triage/duty weeks (approximately one week per month), hybrid working at other times with a minimum of three office-based days per week.

• Social Work England registration and enhanced DBS.
• Strong assessment, risk assessment, and safeguarding skills.
• Experience managing complex caseloads within a fast‑paced team.
• Excellent communication skills and a strong commitment to Equality, Diversity, and Inclusion.
• Relevant experience in front door, hospital discharge, Reablement, or intermediate care services teams.
