Overview
Job Title: Placements / Care at Home Broker
Service Area: Adult Social Care
Location: Kingston (Hybrid – Office attendance required for induction and every Thursday thereafter)
To source, negotiate, and secure appropriate care placements and home care packages for adults with assessed social care needs. The postholder will work closely with social workers, providers, and internal teams to ensure timely, cost-effective, and quality placements in line with statutory duties and council policies.
Key Responsibilities- Receive and review placement referrals from social work teams.
- Identify suitable residential, nursing, supported living, or domiciliary care providers based on assessed needs.
- Liaise with care providers to confirm availability, suitability, and costs.
- Negotiate placement fees in line with agreed frameworks and budgets.
- Coordinate urgent and routine placements, including hospital discharge cases.
- Ensure all placements meet safeguarding, quality, and contractual standards.
- Maintain accurate and up-to-date records on IAS (Integrated Adult System).
- Monitor responses from providers and escalate where necessary to prevent delays.
- Work collaboratively with commissioning and finance teams where required.
- Prioritise workload effectively within a fast-paced, high-demand environment.
Essential:
- Experience working within Adult Social Care.
- Understanding of the Care Act 2014 and adult social care processes.
- Experience arranging care placements and/or home care packages.
- Working knowledge of IAS (Integrated Adult System) or a similar case management system.
- Experience negotiating with care providers.
- Ability to manage multiple urgent cases simultaneously.
Desirable:
- Experience supporting hospital discharge pathways.
- Knowledge of local care markets and provider networks.
- Strong organisational and prioritisation skills.
- Ability to work independently and “hit the ground running.”
- Confident communicator with strong negotiation skills.
- Ability to build positive working relationships with providers and colleagues.
- High level of accuracy and attention to detail.
- Resilient and able to work under pressure.
- Attendance at the office is required for induction and initial training.
- Ongoing requirement to attend the office weekly on Thursdays.
- Hybrid working arrangements for the remainder of the week, subject to service needs.
